Position: Clinical Resource Nurse             

Department: Acute Inpatient Rehabilitation Unit – 4 North

Status:  Full-time

Hours:  Monday to Friday – 8 hour day shifts; weekends as required (Hours subject to change based on operational requirements)

This is what you'll be doing:  

  • Reports on a daily basis to the Patient Care Manager.
  • In collaboration with the Professional Practice Clinician and Patient Care Manager facilitates the ongoing professional needs of Inpatient Rehab staff.
  • Is responsible for the day-to-day management of operational issues on the Inpatient Rehabilitation Unit
  • May be responsible for completing NRS assessments
  • Monitors data collected from NRS assessments and FIM assessments and provides reports
  • Works collaboratively with Rehab colleagues to facilitate the flow of patients within the program.
  • Works with the Patient Care Manager/Professional Practice Clinician to implement and promote revised/new policies and procedures, standards of nursing care.
  • Actively troubleshoots and resolves issues related to the implementation of standards/policies and procedures.
  • Accepts and completes special projects as assigned by the Program Director or Patient Care Manager/Professional Practice Clinician.
  • Assists the Patient Care Manager in monitoring and managing the financial resources of the unit.
  • Assists the Patient Care Manager in ongoing monitoring and tracking of utilization activities of the Inpatient Rehabilitation Unit
  • Assists the Patient Care Manager in the hiring/interviewing process of the Inpatient Rehabilitation Unit
  • Is responsible for the orientation, mentoring, and evaluation of new staff.
  • Assists the Patient Care Manager in the development of continuous process improvement initiatives for Inpatient Rehabilitation Unit
  • Is responsible for ensuring ongoing monitoring of quality indicators and identifying improvement strategies in consultation with the Patient Care Manager/Professional Practice Clinician and the Inpatient Rehabilitation Unit staff to improve patient care.
  • Actively promotes patient/family focused care.
  • Coordinates and facilitates daily team quality and safety huddle
  • Coordinates and facilitates team interdisciplinary rounds

  • BScN or BN required
  • Current Registration with the College of Nurses of Ontario required
  • Previous clinical resource nurse experience preferred
  • Minimum of 3-5 years of clinical experience required
  • Clinical expertise in a Rehab setting required
  • Stroke/Neuro experience or the Hemispheres course an asset
  • Participates on a regular basis in in-service and educational programs/upgrading
  • Demonstrated ability to mentor, support, advise and lead others
  • Models a high level of professional practice related to clinical and interpersonal skills
  • Demonstrates conflict management and conflict prevention and resolution skills
  • Superb problem-solving skills
  • Excellent communication skills both written and verbal
  • Strong organizational skills
  • Acts as a professional role model for his/her nursing colleagues
  • Actively promotes a healthy workplace for all staff
  • Proven success in developing relationships with the interprofessional healthcare team
  • Must be able to meet the physical demands of the position
  • Satisfactory employment and attendance record required

Salary: $34.43 to $48.49 per hour
