This is what you will be doing:

Reporting directly to the Director of Nephrology at Halton Healthcare the OTMH Nephrology Patient Care Manager works in collaboration with the Technical Manager, the Patient Care Manager for the Burlington site, the Program Medical Leadership, and Professional Practice Clinician to implement clinical program priorities and oversee the daily operations and professional practice of the Renal Dialysis Services located at the Oakville Hospital Site and the Acute Dialysis offsite services.

The patient care manager plans, implements and evaluates activities related to the provision of high quality patient care services and programs, selects, hires and develops staff, works collaboratively with the team, and maintains fiscal responsibility. The Patient Care Manager is also be instrumental in leading and supporting the growth in the Home Dialysis services and Pre-Dialysis services, key priorities of the Ontario Renal Network and in supporting partnerships with the ORN, community partners, and the MH-LHIN.

This is what you will need to do it:

  • Current Registration with the College of Nurses of Ontario
  • BScN required
  • Masters preparation or evidence of substantial completion preferred
  • A minimum of 3 years of recent experience as a patient care manager in a similar hospital environment
  • Excellent human resource management skills including recruitment and retention of staff
  • Excellent budgeting and financial management skills
  • Proven leadership skills preferably in Nephrology
  • Excellent computer skills including proficiency with Microsoft Office applications including Word, Excel and PowerPoint
  • Superior verbal and written communication skills
  • Excellent customer service skills
  • Demonstrated ability to support team building
  • Must be able to meet the physical demands of the position
  • Current R.N.A.O. membership preferred
  • Demonstrated commitment to continuing education
  • Certification in Nephrology an asset.
  • Satisfactory employment and attendance record required
