Position: Registered Dietitian

Department: Halton Diabetes Program

Status: Part-time (must provide availability up to 0.4 FTE)

Hours: Monday to Friday only. Days & Evenings 7.5 hour shifts. (Hours are subject to change based on operational needs)

Site:  Multi (Travel between hospital sites as needed)

This is what you’ll be doing:

Provides diabetes self-management education and management to people with diabetes and their families or significant others both individually and in groups. .

  • Assesses referred patients to determine health status, diabetes self-management skills and learning needs, diabetes management and glycemic control.
  • Collaborates with patients to establish learning and diabetes management goals.
  • Monitors and interprets capillary blood glucose, A1C, lipid and blood pressure values to assess diabetes control.
  • Using self-management/motivation interviewing techniques and adult learning principles, facilitates learning with patients and families in group and individual formats.
  • Collaborates with patients and other health professionals to evaluate patient progress toward achieving diabetes management goals, both verbally and through written documentation.
  • Develops nutrition plans appropriate to individual energy needs, activity level and type of medications
  • Demonstrates expertise in diabetes management in all aspects of patient care.
  • Recognizes and deals with medical emergencies that arise in the Diabetes Centre.
  • Contributes to overall program planning and development.
  • Maintains required patient records and statistics.

  • BSc in Food and Nutrition and accredited dietetic internship
  • Registered Dietitian in good standing with the College of Dietitians of Ontario
  • Certified Diabetes Educator Certificate (CDE) preferred
  • Previous experience in diabetes education (individual and group preferred)
  • Experience working with diverse cultural individuals/groups is preferred
  • Multiple language fluency is an asset 
  • Computer literacy (Microsoft Office)
  • Excellent leadership and problem solving skills
  • Excellent oral and written communication skills
  • Demonstrated application of adult education principles, motivational interviewing/self-management techniques and group facilitation skills
  • Able to establish positive and supportive relationships with patients, families and other health professionals
  • Must be physically able to do the job
  • Satisfactory attendance and employment record
  • Possesses a valid driver’s license and access to a vehicle for work purposes.
